How does YouTube decide it is a Short?

YouTube does it on its own: a vertical or square video of 3 minutes or less becomes a Short. There is no setting for it.

Is the first comment pinned?

No. Yaybird posts it as the first comment, but YouTube only lets you pin a comment in YouTube Studio.

Can it mark a video as altered or synthetic?

Yes. Turn on the AI label for the post and YouTube shows its altered or synthetic content label.
